New Patients

Deddington Health Centre welcomes new patients who live within our practice area. Please check that you live within the practice area, referring to the map on the Location Maps menu page.

We have a straight forward registration process, call in at our reception and you will be given a welcome pack which includes a copy of our practice leaflet and a health questionnaire for you to complete. During the registration process you will be asked to complete a registration form for each family member including children. We request that you give us details of each family member's mobile phone number. If you would like to be kept up to date with surgery news by receiving our newsletter, then please also give us your email address(es) when you register.

Once registered you will be offered a general health check-up. If you are on medication, this will be with a GP so that they can add your medication to our clinical system. If you are not on medication, this will be with one of our Practice Nurses. Either, telephone the surgery to make a new registration appointment or call in and speak to a receptionist.

Following registration your previous records will be requested, there is normally a delay of 4 to 6 weeks before they arrive, except if urgent.

The accuracy of information you provide us with during registration is therefore very important.
