Registration

Deddington Health Centre welcomes new patients. We have a straight forward registration process during which you will be offered a general health check-up. Please check that you live within the practice area, referring to the map on the "Location Maps" menu page.

Either telephone the surgery to make a new registration appointment or call in and speak to a receptionist.

Our welcome pack includes a copy of our practice leaflet and a health questionnaire for you to complete. During the registration process you will be asked to complete a registration form for each family member including children.

Following registration your previous records will be requested, there is normally a delay of 4 to 6 weeks before they arrive, except if urgent.

The accuracy of information you provide us with during registration is therefore very important.
