We are endeavouring to show our patients how we take care of their clinical records. Below are links to 2 documents that help to explain how we manage your records. These are written by the NHS and explain how your medical records are now, and will in the future, be managed by anyone working in the NHS.

Following limited local piloting, patients' summary care records are now going to be uploaded to a central national database across England and in some parts of the country this has already commenced (not yet Oxfordshire). Anyone who does not want to have a Summary Care Record (SCR) uploaded onto the central database has to opt out by informing their GP Surgery in writing with a signature for every member of the household who wishes to opt out. Their surgery will then put a code into their records which shows that they have "opted out" and their records will not be uploaded. Please contact the surgery if you want more information about this or use the link below to take you to a pre-scripted letter that you can use to send to your GP to opt out.
