Registration Process

Deddington Health Centre gives a warm welcome to any new patients who live within our practice area.

Our registration process is a straight forward process where you can either print off the paperwork here and complete before you visit the Practice or in person at the Health Centre. When you visit the Health Centre to register you will be given a welcome pack which includes a copy of our practice leaflet and a health questionnaire for you to complete or you can print them off to complete below.

If you are on medication you will need to make an appointment with a GP so they can add your medication to our clinical system.

Following your registration your previous records will be requested. There is normally a delay before they arrive. You can still make an appointment at the Health Centre even if your notes have not yet arrived from your previous Practice. The accuracy of the information you provide us with during registration is therefore very important. The importance of keeping this information accurate is also important within our Health Centre so we also ask for you to please keep us up to date with any other changes in the future.

Please note, if you change your landline or mobile phone number, your address, or your email address please notify the practice as soon as possible so that we can update our records.

How to Register with us

During the registration process you will be asked to complete a GMS1 form, a registration form, request access to online services and make decisions about how we share your data for each family member, including children. If you are registering children under 5 you will need to ask for this paperwork at Reception and bring along your child’s red personal child health record book as it is really useful if you can provide us with up to date immunisation details for your child under 5.

Other Documentation Required

We also need to see the following:

  • Photo identification  – driving licence or passport
  • Proof of your address – a bill with your name and address on it
  • Your NHS number – you can get this information from your previous surgery

If you are from overseas and registering with the NHS for the first time we need to view your passport and any relevant visas.
