During the registration process you will be asked to complete a GMS1 form, a registration form, request access to online services and make decisions about how we share your data for each family member, including children. If you are registering children under 5 you will need to ask for this paperwork at Reception and bring along your child’s red personal child health record book as it is really useful if you can provide us with up to date immunisation details for your child under 5.
In order to start the registration process please complete the following forms by downloading them, completing by hand, and then bring them into the practice:
Download the Patient Questionnaire
Other Documentation Required
We also need to see the following:
- Photo identification – driving licence or passport
- Proof of your address – a bill with your name and address on it
- Your NHS number – you can get this information from your previous surgery
If you are from overseas and registering with the NHS for the first time we need to view your passport and any relevant visas.
